ABSTRACT

Objective To explore the value of elastography strain ratio(SR)combined with breast ultrasound imaging reporting and data system(BI-RADS-US)in the differential diagnosis of breast nodules. Methods A total of 471 breast nodules(from 471 patients)were reclassified by SR combined with BI-RADS-US.With the pathology results as gold standard, the area under the receiver operating characteristic(ROC)curve(AUC)was employed to evaluate the diagnostic performance, and the sensitivity, specificity, and accuracy were compared between the combined method and BI-RADS-US.Results Among the 471 breast nodules,180 nodules were benign and 291 were malignant. The AUC of the combined method was statistically significantly higher than that of BI-RADS-US(0.798 vs. 0.730;Z= 2.583, P= 0.010).SR,BI-RADS-US,and the combined method for diagnosing breast nodules had the sensitivity of 86.6%,99.0%,and 96.6%,the specificity of 67.2%,47.2%,and 63.3%,and the accuracy of 79.2%,79.2%,and 83.9%,respectively.The combined method increased the specificity from 47.2%(BI-RADS-US)to 63.3%(χ

ABSTRACT

Objective To compare the efficacy of ATEC system and conventional surgery for the treatment of nonpalpable breast nodules. Methods Two hundred and forty cases patients with nonpalpable breast nodules who were treated in Maternal and Child Health Hospital of Pinggu District of Beijing from September 2014 to June 2016 were admitted and randomly divided into observation group and control group,120 cases in each group. The observation group was treated with minimally invasive treatment, while the control group was treated by conventional surgery. The postoperative pathologic results, surgical parameters and complications were compared between 2 groups. Results One hundred and eighteen cases were successfully removed and 210 pathological specimens were obtained in the observation group, including 201 benign lesions in 112 cases and 9 malignant lesions in 6 cases. In the control group,208 benign lesions in 113 cases and 11 malignant lesions in 7 cases were obtained from 120 cases. There was no significant difference in the proportion or type of pathology between 2 groups(P>0.05).The operative time, blood loss, scar length and wound healing time were all significantly lower in the observation group than those in the control group((16.72±3.65)min vs.(41.21±5.94)min,(9.45±2.35)ml vs.(28.42±7.30)ml,(3.64±0.85)mm vs.(28.51±6.34)mm,(3.36±0.67)d vs.(6.79±1.58)d;t=38.245,26.891,42.238,21.738;P<0.001).The incidence of breast collapse or deformity in the observation group was lower than that in the control group(0 vs.6.7%,P=0.007),but there was no significant difference in the incidence of hematoma, skin ecchymosis, local infection or nodular recurrence between 2 groups(P>0.05).Conclusion Breast minimally invasive resection is effective in the treatment of nonpalpable breast nodules, and is suitable for patients with high demands on breast appearance.

ABSTRACT

Objective To evaluate the applicated value of color Doppler ultrasonography (CDUS) and CR molybdenum target in the diagnosis of breast nodules. Methods Comparative analysis of 87 patients with breast nodules diagnosed by CDUS and CR molybdenum target were contrasted with postoperative pathology. Results 47 cases of 78 patients with breast nodules were benign lesions.39 cases were correctly diagnosed by CR molybdenum target,41 cases were correctly diagnosed by CDUS,and 44 cases were correctly diagnosed by CR molybdenum target and CDUS.Contrasted with postoperative pathology, the accuracy rates of breast disease diagnosed were 82.5%,75.0% and 95.0% diagnosed by CR molybdenum target,CDUS,CR molybdenum target and CDUS.Conclusion There is no significant statistical difference between CR molybdenum target and CDUS in the diagnosis of benign breast nodules. The accuracy rate of breast disease diagnosed by CR molybdenum target and CDUS is better than that diagnosed by CR molybdenum target or CDUS only. The two co-diagnosis are benefit to raise the accuracy rate of breast nodules.

ABSTRACT

Objective To assess the diagnostic value in breast nodules with ultrasontic scoring. Methods Benign and malignant breast nodules were studied with ultrasonic scoring and compared with pathologic results. Results The 2D-ultra-sontic scoring and total ultrasontic scores of benign breast nodular lesions had significant difference compared to malignant breast nodular lesions (P<0.01).Ultrasontic scores of malignant breast nodular lesions were higher than those of benign breast nodular lesions (P<0.01).According to the ROC curve and Youden index,2D-ultrasonic scores ≥11 could be considered as the critical point for the diagnosis of malignant breast nodular lesions, the total ultrasonic scores ≥12 could be considered as the critical point for diagnosis of malignant breast nodular lesions. Conclusion Ultrasonic scoring for breast nodular lesions may be useful for improving the diagnosis accuracy of the benign and malignant breast nodular lesions.
